I conduct volunteer and child interviews, enter interview information into the AIM data base, work with school counselors to obtain referrals, and help with community events for our volunteers and children.

Meet with residents of the shelter to help them update/create resumes, apply for jobs, and enroll in various therapy classes. Also go out in the field to deliver lunches to the unsheltered homeless

Monitor progression of community services that they are receiving

process cases within a timely manner; determine eligibility for all medicaid programs; assist with local and state hearings

Supervise and lead a group of 15 child welfare specialists on the statewide abuse hotline. Audit referrals, complete performance evaluations, staff child abuse cases weekly on multidisciplinary team, hold monthly staff meetings and conferences.

I have a split position: 1) I work 1/2 time for Crisis Intervention providing short-term case management, information and referral, in-home crisis counseling and advocacy for individuals w/ intellectual and developmental disabilities and their families. 2) I work 1/2 time in the Article 16 clinic conducted Psychosocial Evaluations, Mental Health Screenings and Individual Psychotherapy w/ individuals w/ intellectual and developmental disabilities and their families.

Meet with low income customers to administer a Federally Funded Energy assistance grant, process Senior Rate Credits, assess low income customers for various payment programs, connect low income customers to various public resources to help meet basic needs and do outreach to low income families.

I provide both individual counseling and family counseling to families who are at risk of having their children removed by the state. My job can also involve working with adolescents who are involved in the juvenile court system and who are considered "at-risk" teenagers. My responsibilities include maintaining regular contact with schools, therapist, and community agencies to coordinate services. I also need to complete a treatment plan that I will use with the family or individual during the period of service delivery. I meet with clients 1 to 2 times per week for a period of 3 to 6 months. All session are held in the clients home, school, or out in the community. During this time we may work on skill building, therapeutic interventions, connecting to community resources, and other tasks as necessary.

Checking up on families at their homes, ones that have previous and/or current family services cases, helping them with whatever they need help with, such as cooking, cleaning, etc

I am responsible for assisting the court in resolving divorce disputes and mediation if possible. I also suerpvise 10 family counselors.

I am an in-home counselor for the Family Preservation Program. I provide situational counseling as well as case management services to 13 families.

I handle the entire matching gifts process for the Northeast section of the organization.

I provide support and counseling for Seniors and their families. I also provide resources and referals to outside services.

I provide assessment of the client to see what entitlments they are able to apply to. i help with SSI applications, Housing , and ACd voucher Day car services, legal services, summer camps and different types of services.

I provide intensive in home counseling for at risk children and their families, with the goal of preventing the child from being removed from the home or reunifying children that have already been removed. My maximum case load is 5 families, which allows me to provide the intensive attention that my families need. I utilize MultiSystemic Therapy in my job and

PROVIDE MENTORING SERVICES TO YOUTHS BETWEEN AGES 8-18 AT HOMELESS SHELTERS, FAMILY SERVICES SHELTERS, AND JUVENILE JUSTICE SERVICES. TUTORING IN READING AND MATHEMATICS, ASSISTANCE IN FINDING JOBS FOR THOSE WHO ARE ELIGIBLE.
